In order to complete your online application, you will need to be able to upload the following documents:

• A government-issued photo ID, for example: a driver’s license, a passport, or a similar form of government-issued photo ID.  Expired forms of identification will not be accepted
• A recent color photograph of yourself.  Your photo must be taken from the neck up in front of a solid and light-colored background.  You cannot be wearing a hat, face covering or sunglasses (religious head coverings are acceptable).
• Documentation of disability as listed within the application.

After filling out this application you can receive your card by mail or by visiting the CharlieCard Store.  If you choose to have the card mailed, you will receive the card two to four weeks following approval.  If you choose to pick up the card at the CharlieCard store, you will receive an email or phone call from the MBTA with instructions.
